Chaintech GA-5465

This card is based on the Cirrus Logic GD5465 chipset which is better known as the Laguna3D chipset. These cards are somewhat rare since they seem to have been sold in low quantities.

An interesting twitch is that these cards use the uncommon yet expensive (especially in it's day!) RDRAM.

I haven't tested or benchmarked a card like this. It should be DX5-compliant but drivers, especially the early ones, are probably not that good.
