Helios 3D Voodoo 
Just like this CT6670 but with US-retail box and a different PCB color. Back in the day the Creative and Diamond cards were well known. Many game magazines featured reviews and commercials for these cards in the 1998.
On the last picture you see a date-code 1898. This means 18th week of 1998. Not the earliest Voodoo2 you can find out there but certainly not the latest on. If you had this card in April 1998 you would have top of the line 3D graphics on your desktop
